Transportation Assistance

It is the policy of White Mountain Community Health Center to assist patients when transportation difficulties present a barrier to accessing medical care. Click on the categories below for information about transportation services available in our area. 

Information for patients with Medicaid:
Medicaid covers transportation costs to routine appointments when arranged ahead of time with the insurer, with at least 48 hours' notice.  This includes paying for gasoline to travel to a Medicaid-covered service and reimbursement for transportation provided by a patient's friend or family member.  Patients with straight Medicaid, NH Healthy Families or Well Sense can do this by calling Medicaid at (800) 852-3345 ext. 3770, NH Healthy Families at (866) 769-3085, or Well Sense at (855) 739-4775.  Maine patients can find the right number to call for their town of residence here.

Patients with straight Medicaid, NH Healthy Families or Well Sense can arrange for transportation to be covered by calling Medicaid at (800) 852-3345 ext. 3770, NH Healthy Families at (866) 769-3085, or Well Sense at (855) 739-4775. Patients must provide their date of birth and have their Medicaid number on-hand when requesting services. Additionally, the patient must provide the facility name, address, phone number, and reason for appointment. The Gibson Center

Albany, Bartlett, Chatham, Conway, Eaton, Jackson, Madison, North Conway On demand door-to-door transportation for seniors and persons with disabilities for non-emergency, medical, nutritional, employment, and daily living needs. Van runs morning only, Monday through Friday (except holidays or inclement weather).  Clients must be age 60 or older, and be independently mobile (either on foot or in wheelchair) and be able to carry their own items, or travel with an attendant. 48-hour notice required. No fare charged. Suggested donation $3 to $5. 

Caregivers of Southern Carroll County & Vicinity

Alton, Ossipee, Tuftonboro, Wolfeboro Transportation for medical appointments only. There are no limits on intended destination. Caregivers is open 8 a.m. to 4 p.m., Monday through Friday. Clients must reside in one of the four service area towns listed. 48-hour notice required. Free of charge. Donations accepted.

Blue Loon Transit

Albany, Chocorua, Conway, Effingham, Freedom, Center Harbor, Laconia, Madison, Moultonborough, North Conway, Sandwich, Tamworth, Wolfeboro, West Ossipee Door-to-door for elderly or disabled. Transportation not availble beyond Memorial Hospital or Madison. The Flex Route will deviate 1/4 mile. Routes and times are subject to change, so call or visit website for most recent information.

Blue Loon Flex Route - Serves Conway area to West Ossipee, Thursdays only from 9 a.m. to 5 p.m. Riders are encouraged to call ahead for pickup times at locations between scheduled stops. 

Door 2 Door - Trip scheduling must be done by noon the day before the trip. Return trips must be scheduled one hour before the system stops operating. Depending upon location, this service is available Monday through Friday, 8 a.m. to 5 p.m. Pick up times vary; drivers will wait no longer than five minutes.

Long Distance Medical Program - Serves the Tri-State area. Provides transportation to medical facilities for non-emergency medical care. Individuals under age 60 and those not eligible for Medicaid are required to pay out-of-pocket.
Clients must call to be qualified for disabled services. 24-hour notice required. Varies by service and destination. Free services available to individuals age 60 or older and disabled (when funding is available). Patients with disabilities should call Blue Loon to see if funding is available to cover their fare.

Tamworth Caregivers

Tamworth Only serves residents of Tamworth. Transportation to appointments and shopping. Delivery of prescriptions and library books. Based upon availability of volunteers, Monday through Friday. Destination dependent upon volunteer.

A quick enrollment over the telephone is all that is required to access services. Passengers must be independently mobile. 48-hour notice required.

Interlakes Community Caregivers, Inc.

Center Harbor, Meredith, Moultonborough, Sandwich Volunteer drivers provide local transporation for medical appointments, shopping, and other support services. Trips are based on volunteer availability. There are no limits on intended destination; long-distance medical trips will be accommodated, if possible. Volunteers are not medically-trained.  Clients must reside in one of the four service area towns listed. Clients must be independently mobile or able to self-transfer from wheelchair. 48-hour notice required. Free of charge. Donations accepted.

Located in Albany and Ossipee. Serves Carroll County/Statewide. Non-emergency local and long-distance ambulance and wheelchair van transport. Wheelchair van transport is covered by Medicaid. Patients with straight Medicaid, NH Healthy Families or Well Sense must arrange services through their insurance. Fees vary with service. Base rate is $30 one-way, $60 round-trip; mileage is calculated at $3.50 per mile.

American Cancer Society

Statewide Rides to and from cancer treatment center using a network of volunteer drivers operating their own vehicles. Based upon availability of volunteers. Patients must be traveling to an appointment required to begin or complete cancer treatment, or to an appointment for complementary therapy during cancer treatment. Patients must be ambulatory, able to walk unassisted to and from the vehicle, or have an accompany caregiver to assist. Patients under the age of 18 must be accompanied by an adult. A four (4) business day advanced notice is required, not including the date transportation is needed.  None required.

Veterans Administration - Veterans Transportation Program

New Hampshire Veterans Transportation Service - Provides safe and reliable transportation to veterans who require assistance traveling to and from VA health care facilities and authorized non-VA health care appointments.

Beneficiary Travel - Program reimburses eligible veterans for costs incurred while traveling to and from VA health care facilities. 
   Free to veterans.
