Social Workers
WMCHC offers all aspects of social work to our patients. The Prenatal Program has a social worker to assist in applying for Medicaid, accessing resources in the community, and other forms of support as identified by the patient and the prenatal team.
A full time social worker is available to pediatrics and adults. The pediatric role includes routine developmental screenings, assistance with applying for Healthy Kids and other programs, parenting support, advocacy, resource and referral as needed. General social work includes assistance with transportation needs, applying for services on the town, state and federal levels, advocacy, resource and referral. The American Cancer Society "Freshstart" smoking cessation program is available by appointment with our social worker and at no charge to our patients.
Food Pantry
At WMCHC, we recognize the importance of an adequate diet as a key to our patients' general health. Unfortunately, some of our patients do not always have the means to provide an adequate supply of food to either themselves or their families. That's where our food pantry comes into the picture. Our tiny pantry provided over 3,000 meals in 2008. Additionally, under the direction of our nutritionist, a section of the pantry is set aside for the specific needs of our diabetic patients.
Partners in Health of Carroll County
WMCHC is the home base for this county-wide organization, which assists families with a child that has a chronic illness. Examples of such illnesses are diabetes, leukemia, asthma, Cystic Fibrosis or other debilitating conditions.
The Family Support Coordinator collaborates with the family and the community to assist with resources and information. Limited funds can be available for financial aid for medicine, emergency food, transportation, summer camp and recreation, and other needs. There is no cost to the family regardless of income.
This program is funded by the New Hampshire Department of Health and Human Services and is administered through the White Mountain Community Health Center.
